Homocysteine (HCY) is a thiol-containing amino acid produced in the degradation of methionine. Homocysteine exported into plasma circulates mostly in its oxidized form, bound to plasma proteins. Smaller amounts of reduced and disulfide forms are also present. Total homocysteine is the sum of all homocysteine forms present in plasma or serum. The metabolism of homocysteine is regulated by three enzymatic pathways and the pathways need vitamins (folic acid, vitamin B6 and vitamin B12) as cofactors. Genetic disorders of enzymes involved in homocysteine metabolism results in homocystinuria. Deficiency of vitamins is a cause of elevated homocysteine levels in plasma or serum. Studies have shown that the homocysteine concentrations in blood are associated with the risk of cardiovascular diseases (CVD). It has been suggested that increased homocysteine levels are an independent risk factor for coronary artery disease.
