Presepsin is the subtype of the soluble form of lipopolysaccharide receptor, CD14 (sCD14-ST). Presepsin is the 13 KDa (64 amino acids) N terminal fragment of soluble form of CD14 (sCD14) which is a member of the Toll-like receptors (TLRs), cleaved by cathepsin D in plasma, and involved in activating the innate immune system. Presepsin is normally present in very low concentrations in the serum of healthy individuals. In response to bacterial infections, its concentration increases within 2 hours, according to the severity of the disease. Presepsin is more specific for bacterial infection due to its direct involvement in the pathomechanism of infection. Presepsin has good accuracy in discriminating sepsis from non-infectious organ failure and in discriminating septic shock from sepsis. Presepsin has been demonstrated to be a useful inflammatory indicator for sepsis having high sensitivity and diagnostic accuracy in diagnosing sepsis. As it has high sensitivity, presepsin may be very useful for the exclusion of sepsis in pediatric patients when the level of this biomarker is normal or lower than the cutoff value. Presepsin has a higher prognostic indicator of sepsis and helps in assessment of sepsis severity.
